- If your Pap smear shows evidence of cervical dysplasia, your doctor may decide to take a look inside your cervix with an instrument called a colposcope. This instrument has a magnifying lens and is used to identify any abnormal cells that might need to be examined in a biopsy.
- If colposcopy or biopsy shows precancerous changes, the next step is to destroy the abnormal cells. In many instances, this can be accomplished with heat, by laser or cauterization, or with cold, by cryosurgery. These methods destroy abnormal cells and are performed on an outpatient basis. These are few side effects and healing is usually rapid.
- Cone biopsy therapy is used to diagnose and remove abnormal tissue. In this procedure a cone-shaped tissue sample is surgically removed from the cervix under general or spinal anesthesia in an operating room. The specimen is sent to a pathologist for detailed microscopic examination. If dysplasia is found, no further therapy is needed, provided all of the affected cells were removed. If cancer is found, additional treatments are indicated. The cone biopsy is usually performed for high-grade dysplasia, dysplasia that recurs, and suspected cancer.
- A relatively new technique called the loop electrosurgical excision procedures (LEEP) developed in Britain, offers advantages over a cone biopsy. LEEP allows the removed tissue to be examined by a pathologist to ensure that the lesion was completely removed and provide an accurate assessment.
- The Pap smear is used to screen for cervical changes. Although Pap smears effectively reduce the incidence of cervical cancer, there is an ongoing debate about how often the test should be performed. Guidelines and recommendations vary.
