Treatment begins with the search for the cause. This can be difficult, but it is worth the effort. If fungal, bacterial, or parasitic infection is involved; for example, it is difficult to clear a dermatitis problem without addressing the cause.
Steroids, whether oral or topical, are a mainstay of treatment for most types of dermatitis. Examples include cortisone, fluocinolone, prednisone, and triamcinolone. The use of these drugs must be carefully monitored. Because they can be dramatically effective, it can be tempting to continue them for long periods, but this can lead to a breakdown of normal skin defenses over time, with resulting changes in skin texture and color. Also, cortisone is absorbed into the body even with topical use, which can cause problems resulting from suppression of adrenal function over time. Steroid use is also associated with an increased risk of glaucoma.
If steroids alone are not adequate, an antifungal and/or antibacterial drug may be tried. Theses can be taken topically or orally.
Antihistamines such as diphenhydramine (Benadryl) or hydroxyzine (Atarax, Vistaril) may be effective in controlling the itching of many forms of dermatitis. These too may be taken orally or applied topically. The oral forms can cause drowsiness. However, that side effect may be useful if you are having difficulty sleeping due to severe itching.
For some types of dermatitis (not the photoallergic variety), exposure to ultraviolet light may help the skin clear considerably, especially on the hands and feet.
In severe cases, the drug isotretinoin (Accutane) may be tried. This drug can have serious side effects, however, including increased cholesterol and blood sugar levels, liver damage, kidney damage, and blood-chemistry changes. It must never be taken by a woman who is pregnant, or who might become pregnant, as it is extremely damaging to a developing fetus and can cause birth defects.
For stasis dermatitis, treatment includes elevation of the affected extremely above the level of the heart to assist in returning fluids to the heart.
